Job Title: ENHANCED CARE MANAGER
Job Summary: The Enhanced Care Manager will be responsible for coordinating comprehensive care services for clients, focusing on improving their overall health and well-being. This role includes developing individualized care plans, assessing clients’ needs, coordinating with healthcare providers and social services, and advocating for the client’s access to necessary resources. The Enhanced Care Manager ensures seamless integration of medical, behavioral, and social support services to promote long-term stability and recovery.
Job Title: HOUSING NAVIGATOR
Job Summary: The Housing Navigator will assist clients in identifying, securing, and maintaining stable housing. This role involves working closely with individuals to assess housing needs, provide guidance on available housing options, and support clients through the application and leasing process. The Housing Navigator will also collaborate with landlords, housing authorities, and service providers to ensure clients have access to necessary housing resources and support.
